Patient Forms

No one likes filling out form after form at the practitioner’s office. So, why not complete your paperwork beforehand?

Our forms are “fillable” PDF files, meaning they can be filled-in using your computer. Once completed, you may either save and submit them to us or print and bring them to your first appointment. If you prefer, you may also print and complete them by hand.

New Patients

If you're a new therapy patient, please complete our Patient Registration and Patient Health Status forms prior to your first appointment. This saves time at your first appointment and helps us understand your current physical well-being, which will enable us to give you the best, most informed care.

Patient Consultations

If you've scheduled an appointment for a free consultation, please complete our Patient Consultation form. This will help the therapist learn more about your condition and allow more time for the initial evaluation.

Please note: If you chose not to complete the appropriate form(s) in advance, arrive at least 30 minutes before your first appointment.
